WE NEED YOU
New Hall Surgery has set up a patient group. It meets 4 times per year. Our aim is to have a Patient Group, representative of all patient age groups and needs within the surgery. Ideally, we would like to set up a group across all age ranges, with other groups represented, such as carers, mums & dads and older people.


If you are a young person, in your teens we would also be interested in talking to you.
If you would be interested in taking part in the Patient Group, please drop your details off at reception and a member of our team will contact you. Your commitment would only be about 1 hour, three or four times a year.

We have also set up a 'virtual' group, by which I mean a list of patients we can contact by email to run ideas past them, from time to time. If you are interested in signing up please leave your name and contact details at reception.

If anyone out there feels that they have a particular skill, interest or leadership qualities in this area we would particularly like to hear from you.


We have agreed a Patient Group Terms of Reference and have also undertaken a Patient Survey during 2013/14. We collect patient comments and suggestions in a number of ways and, where appropriate, act upon them. We run a Friends & Family Test within the surgery, details of which are now available on this website. There is also a plan of action for improvements, which has been agreed and put together in conjunction with our Patient Participation Group.

NEW HALL SURGERY PATIENT PARTICIPATION GROUP TERMS OF REFERENCE

Title of the Group
The group shall be called the New Hall Surgery Patient Participation Group (PPG).

Aims of the Group
The aims of the Group are to promote co-operation between the practice and its patients, and
contribute to the continuous improvement of services.


Membership of the Group
Membership of the Group is limited to patients permanently registered at the practice.
The Group will, as far as possible, be a representative mix of the practice population.  The PPG will be supplemented by a Virtual Patient Group
Membership of the Group will be limited to 20 patient members at any time. If the number of patient representatives falls below 10, additional members will be sought.
The practice reserves the right to decline the proposed membership or remove the membership of any individual.  Reasons will be given.

Activities of the Group
This PPG will:
Work with the practice on service development and provision.
Contribute to, and be kept informed of, practice decisions.
Represent the views of the patient body and move towards providing feedback on their needs and concerns.
Contribute to the design of and participate in the review of the practice patient survey.
Membership will be reviewed on an annual basis.

Meetings
Venue is to be the Health Promotion room at New Hall Surgery.
There must be a minimum quorum of 4 Group members to render a meeting valid.
The Group will endeavour to meet no fewer than four times a year, although initial meetings will be more frequent.
A member of practice staff will attend every meeting.

Organisation of the Group
The Group will be chaired by a member of the practice and supported by the practice staff.  Practice staff may be invited to attend where appropriate
Administrative support to the Group will be provided by the practice.

Reporting
Minutes will be circulated to group members after each meeting and made available in reception and published on the PPG web page once approved by the Chair.

New Hall Surgery plan of action 2014 - 2015:
The following plan of action was discussed, formulated and agreed by the PPG.
Priority Area 1:
Patient confidentiality at front desk - this had been a priority the previous year and it was carried forward to ensure that patients were happy with improvements and did not require more to be done
Priority Area 2:
   Practice Signage improvements

Priority Area 3:
   Work with patients to provide and improve on-line electronic access to appointments, ordering of prescriptions and access to the patient record (March 2015).  Also monitor and improve access for patients via the practice telephone system
